Registration Process
To register with the practice you can either complete the Registration form and the Patient Questionnaire attached below or pick up forms from the surgery.
Once you are registered you will be offered a new patient medical appointment and asked to bring a urine sample with you to the appointment.
Please note that when you register it is with the practice rather than with an individual GP but you have the right to express a preference as to which GP you wish to attend, simply tell the receptionist when you make your appointments.
